Newly Graduated Seniors! You must order your official Antioch University transcript for any college classes you took during your time here (US History 101 and 102 Junior year, and Art History Senior year) be sent to the 4 year university you will be attending in the Fall. DV Design/Antioch will not be doing this- YOU need to request it! For more info, click on the link below:

Please check off “First Transcript Order- Complementary Transcript Packet” in the above form. Please make your request after June 22nd to allow your Art History grades and comments to post.

UC deadline for documents: July 1st

CSU deadline for documents: July 15th

Private/other colleges: vary- check with your college directly (assume July 1st to avoid any confusion)

**If your college is requiring you to submit your college transcript and you do not submit your Antioch transcript by the college’s deadline, your admission can be revoked*
