DVD Summer School Registration Form
DVD students and families, below you will find the 2016 Summer School information and registration form. There will be 2 sessions offered (each session is one semester’s worth of credit); June 13-30 and July 5-22. If you need to recover course credit through summer school, please complete the form below and return to the front office.

A few notes:
- You may have already heard from your counselor about this if you are missing credits from previous semesters.
- If you have not yet heard from a counselor, but you know you are missing credits, we recommend that you turn in a form to reserve a spot in summer school and/or contact your counselor.
- If you currently have an “I” or incomplete in a class this semester, we recommend that you turn in a form to reserve a spot in summer school should you need it (if you pass the class by June, you will be removed from the roster).
- Please carefully review the form and the attendance policies, as they are very strict during summer school due to the short length of the courses.
